The mathematical approach to the Atlas Subluxation Complex (ASC)
The NUCCA procedure is distinguished by its mechanical reduction protocol based on the physics of levers. Unlike qualitative approaches, our intervention is purely quantitative.
Precision orthogonal imaging: We use a series of three radiographic views (Nasium, Vertex, Lateral) with strict cephalometric alignment. This protocol eliminates rotation artefacts and parallax errors, allowing measurement of structural misalignment relative to the central orthogonal plane with an error margin of less than 0.25 degree.
Correction vector calculation: Misalignment is not treated as a simple translation but as a combination of vectors in all three planes of space. The correction vector is calculated from multiple factors including coronal inclination of C1, head and cervical spine, rotation of C1 and C2 in the transverse plane, and the anatomical configuration of the occipital condyles and superior articular surfaces of C2.
Minimal force and inertia: The NUCCA adjustment uses light, controlled, vectorized pressure applied by the chiropractor's hands between the mastoid process and the transverse process of C1. This force is insufficient to trigger the protective reflex of the paraspinal muscles, allowing passive and stable reduction of craniocervical junction subluxation.
Post-correction validation: Immediate follow-up radiographic views are taken to confirm the reduction trajectory after the first correction. This feedback loop is unique in manual therapy and ensures reproducibility of results.

Impact on fluid dynamics and neuromodulation
For neurologists and neurosurgeons, the C0–C1–C2 junction acts as a bottleneck: any misalignment can affect cerebrospinal fluid (CSF) hydrodynamics, venous drainage and glymphatic function.
CSF hydrodynamics: Atlas misalignment can induce relative stenosis at the foramen magnum, disturbing pulsatile CSF flow and intracranial pressure. Phase-contrast MRI (PC-MRI) studies, including those by Dr. Raymond Damadian, have shown that correction of the craniocervical junction improves CSF flow velocity and compliance, offering a therapeutic avenue for chronic migraines and refractory tension-type headaches.
Venous drainage and glymphatic system: The glymphatic system is the mechanism by which CNS waste (including proteins such as beta-amyloid) is cleared via perivascular spaces, in conjunction with CSF flow and arterial pulsatility. Impaired CSF flow at the craniocervical junction (foramen magnum, compliance) can compromise glymphatic clearance and promote metabolite accumulation, with implications for post-traumatic recovery, chronic headaches and potentially neuroinflammation. Improvement in CSF flow after correction of the craniocervical junction (as documented by PC-MRI) may therefore support better glymphatic function.
The myodural bridge (RCPM): The Rectus Capitis Posterior Minor muscle has a direct attachment to the spinal dura mater. A C1 subluxation creates direct mechanical tension on the dura, causing meningeal irritation and diffuse neurological symptoms often mistaken for idiopathic neuralgias. Correction of the ASC can relieve this tension.
Trigeminocervical convergence (TCN): The TCN refers to the region where nociceptive input from the upper cervical roots (C1–C3) and the trigeminal nerve (V) converges in the cervical cord and brainstem. This convergence explains why irritation of the craniocervical junction can present as orofacial pain, vascular headache or neuralgia, and why cervicogenic headaches or trigeminal neuralgia may have an ascending cervical component. Correction of the ASC can modulate this convergence and reduce nociceptive facilitation.

Reflex spasticity correction and autonomic regulation
Correction of the ASC can influence vestibulospinal and reticulospinal pathways involved in postural tone. High-origin spastic contracture can present as pelvic tilt or functional leg length discrepancy. Posturometric evaluation (postural inclinometer, pelvis and shoulder alignment) helps objectify changes before and after correction.
Autonomic neuromodulation and hypertension: Precise atlas correction affects microcirculation at the brainstem level and the nucleus tractus solitarius (NTS), the central regulator of the baroreflex. The Bakris study (University of Chicago), a randomized double-blind trial in 50 hypertensive patients, showed at 8 weeks a systolic reduction of −17 ± 9 mm Hg vs −3 ± 11 mm Hg (p < 0.0001) and diastolic −10 ± 11 mm Hg vs −2 ± 7 mm Hg (p = 0.002), comparable to two-drug antihypertensive therapy.

La fondation cervicale de l'équilibre cranio-mandibulaire
The cervical foundation of craniomandibular balance
Upper cervical alignment is the foundation of occlusion. Stable occlusion depends on a horizontal cranial base; a dentist cannot stabilize a TMJ if the cranial base is tilted.
Temporal plane horizontality: An atlas tilt alters the inclination of the temporal bone and thus the mandibular fossa. The condyle adapts to this asymmetry, potentially causing joint sounds, opening deviations and premature wear. Correction of the ASC can restore a reference base for occlusal and TMJ evaluation.
Cervicomandibular proprioception: The alar ligaments and neuromuscular spindles of the suboccipital region inform mandibular position and head posture. C0–C1–C2 dysfunction can bias this proprioception and complicate orthodontic or prosthetic work. Trigeminocervical convergence explains why junction irritation can mimic or exacerbate orofacial pain or TMJ disorder.
Pre-surgery collaboration: We encourage evaluation of the craniocervical junction before major orthodontic treatment or maxillofacial surgery, so that correction is built on a stable foundation.

Synergie entre alignement structurel et mobilité fonctionnelle
Synergy between structural alignment and functional mobility
The craniocervical junction is the top link of the kinetic chain. Atlas misalignment (ASC) influences global posture, cervical and scapular muscle tone and the biomechanics of lower segments. Physiotherapists and osteopaths address mobility, strength and compensations; the NUCCA procedure targets the foundation (C0–C1–C2) with minimal-force, measurable correction (pre/post X-rays). Correction of the ASC can reduce high-origin tension (myodural bridge, suboccipital muscles), allowing manual therapy and exercise to address tissues that are less guarded. We encourage collaboration: sharing of reports (junction measurements), cross-referral for vertigo, chronic neck pain, post-concussion syndrome and cervicogenic headache.
Inhibition of righting reflexes: Righting reflexes maintain head and body orientation relative to gravity. Biased proprioceptive or vestibular input at C0–C1–C2 can sustain compensation patterns. Correction of the foundation can inhibit these maladaptive reflexes and stabilize physiotherapy and osteopathy outcomes.
Care stability: Addressing mobility or myofascial issues without correcting junction subluxation often yields temporary results and frequent recurrence. Once the foundation is stabilized, manual therapy and exercise can act within a more favourable therapeutic window.
Therapeutic window: Suboccipital proprioception and myofascial release respond better after ASC reduction. We recommend a sequence: NUCCA assessment and correction, then stability and movement reinforcement in collaboration with the physiotherapist or osteopath.
